I agree with the suggestions made by Anne & Chris. One more to consider: for 
Q2, would you want to limit responses to 2 or 3, or alternatively, use a 
ranking option and have people rank them from 1 (most impt) to 6 (least impt)?

Hi,
Also in #2, could "staff" be considered limited to non-librarians/faculty?
I use the word "personnel" to be inclusive of both groups.
In #3, would "offered by ASERL" be better than "moved to the ASERL level"?
